Clarion Forest VNA is seeking dedicated and compassionate full-time Licensed Practical Nurses (LPN) to join our Home Health team. This role involves providing skilled nursing care to patients in the patient's home under the supervision of a registered nurse (RN) in accordance with physician orders and patient’s plan of care.
Responsibilities:
- Conduct thorough ongoing assessments of individualized patient care plans initiated by RN
- Provide nursing interventions, therapeutic procedures, and education to patients and their families.
- Collaborate with physicians and team members to optimize patient outcomes
- Manage and document patient care thoroughly and accurately, ensuring compliance with all regulations
- Maintain a professional approach to patient and family interactions
Qualifications:
- Graduate of an accredited nursing program
- Active LPN license in the state of Pennsylvania
- Have a minimum of 2 years of acute care or long-term care experience: previous home health experience preferred
- Ability to pass PA Child Abuse History, Criminal Background, and FBI Check
- Valid PA driver’s license and reliable, insured transportation
- Current CPR certification
- Strong communication, organizational, and critical thinking skills
Physical Requirements:
- Ability to perform tasks such as walking, bending, and lifting up to 50 lbs
- Comfortable working in various home environments and settings
- Visual and auditory acuity sufficient for written and verbal communication
Hours are primarily Monday-Friday 8:00AM-4:00PM plus on average 1 weekend/month and 2 Holidays/year.
Starting hourly wage commensurate with education and experience.
